Join Our Caring CommunityHumana's Primary Care Organization is a provider of senior-focused, value-based care, with more than 340 centers across 15 states under the CenterWell and Conviva brands. This is a full-time, on-site, patient-facing role. You report to the Associate Medical Director and are responsible for both in-visit care and ongoing patient management.
