We're seeking a compassionate Occupational Therapist to provide one-on-one home health services in the Great Neck area, helping older adults improve function, confidence, and independence.

What You'll Do:

• Perform occupational therapy evaluations in patients' homes.
• Design individualized treatment plans based on each patient's functional goals.
• Address limitations affecting self-care, mobility, and everyday activities.
• Recommend adaptive equipment, home modifications, and safety strategies that support independent living.
• Educate patients, family members, and caregivers on techniques that promote long-term success.
• Monitor patient progress and update treatment plans as needed.
• Complete timely electronic documentation in accordance with Medicare and agency guidelines.

Qualifications:

• Current New York State Occupational Therapist license.
• Strong clinical reasoning and communication skills.
• Ability to work independently in a community-based setting.
• Experience with adult or geriatric patients is preferred but not required.
• Home health experience is welcome but not necessary.
