Case Manager - Inpatient Discharge & Care Coordination

Select Specialty Hospital is seeking a Case Manager for a full-time, Mon-Fri 8-5 role at our LTACH. Requires licensure in a clinical discipline (RN/LPN/LVN or RT) or Social Work per state guidelines; inpatient discharge planning experience preferred. You will lead utilization reviews, discharge planning, treatment plan and financial management, document medical records, and collaborate with the care team to ensure timely, patient-centered outcomes. #J-18808-Ljbffr Select Medical
