Work Environment\n

This role is designed for a remote work setting, providing flexibility while still ensuring that essential services are delivered effectively to our clients. The primary work hours will be Monday through Friday, from 8:00 AM to 4:30 PM; however, some flexibility is required to accommodate client visits and ongoing support needs. Occasionally, you may be called upon to work evenings or weekends to address urgent concerns or to ensure continuity of care for our clients. An on-call availability may also be necessary to handle pressing situations or client emergencies.

Key Responsibilities\n

• Conduct in-depth psychosocial evaluations of seniors, considering mental, emotional, and social factors influencing their health and well-being.

• Develop and implement individualized care plans in collaboration with clients and their families, taking into account their specific goals and preferences.

• Provide counseling and emotional support, equipping seniors and their families with coping mechanisms to navigate life transitions and challenges.

• Advocate for clients’ rights and help them access community resources essential for their care and support.
\n
• Collaborate regularly with an interdisciplinary team, including nurses, caregivers, and other healthcare professionals, to ensure integrated care delivery.

• Conduct routine home visits to assess living conditions, monitor client well-being, and address safety concerns or needs for additional support.

• Facilitate effective communication and problem resolution among family members involved in the care of the senior client.

• Educate clients and their families about available community resources, support groups, and programs aimed at enhancing their social engagement and overall quality of life.

• Maintain accurate documentation in compliance with organizational guidelines, reflecting client assessments, care plans, progress, and interactions.

Compensation and Benefits\n

The annual salary for this position ranges from $60,000 to $100,000, commensurate with experience and qualifications.
