Communicate ongoing patient status and treatment plans with healthcare team members. Administer medications using advanced bar-scanning and documentation technologies. Document patient assessments, plans of care, interventions, and evaluations electronically. Provide patient and family-centered education for health and wellness optimization. Participate in multi-disciplinary discharges to ensure safe transitions of care.
