Key Responsibilities

• Conduct thorough patient assessments and medical histories to ensure accurate diagnosis and treatment planning.
• Collaborate with physicians and various healthcare professionals to develop comprehensive care plans tailored to individual patient needs.
• Perform full physical examinations, order and interpret diagnostic tests, and create actionable treatment plans.
• Administer vaccinations and medications, educate patients regarding preventive health measures, and recommend lifestyle changes.
• Monitor and manage chronic health conditions, coordinating necessary referrals to specialists when appropriate.
• Ensure precise and timely documentation of patient encounters using the EPIC EMR system to maintain accurate medical records.
• Participate in quality improvement initiatives focused on enhancing service delivery and patient satisfaction.
• Emphasize preventive care strategies to improve patient wellness and educate families about healthy practices.
• Build lasting, compassionate relationships with patients and their families, providing patient-centered care at all times.

Patient Population / Client Focus

Qualifications

Compensation and Benefits
