The work setting is a busy onsite rehabilitation center where you will engage with patients during peak operational hours. Key Responsibilities

• Conduct thorough initial assessments of new patients to determine their physical therapy needs and develop personalized treatment plans accordingly.
• Provide daily therapy sessions for up to 7 patients, focusing on helping them achieve their mobility and rehabilitation goals through both hands-on therapy and guidance on exercises.
• Educate and inform patients and their families about their conditions, treatment progress, and home exercise programs to ensure continuity of care outside the clinic.
• Collaborate closely with a multidisciplinary healthcare team, including physicians, occupational therapists, and nursing staff, to streamline patient care and enhance outcomes.
• Maintain precise and current documentation of all therapy sessions, ensuring compliance with healthcare regulations and facilitating optimal inter-departmental communication.

Qualifications

• A valid Physical Therapist license issued by the state of New York.

• Experience in a skilled nursing facility or outpatient rehabilitation center.

Compensation and Benefits
