As a Licensed Professional Counselor at our organization, you will engage with clients through a telehealth setting, leveraging technology to provide effective care remotely. Key Responsibilities\n

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues, contributing to a comprehensive care team.

• Conduct patient intakes focusing on detailed diagnostic and functional evaluations, then develop tailored treatment goals and care plans.

• Provide brief, focused interventions for patients in need of immediate mental health services, facilitating timely support.

• Advise primary care providers on appropriate referrals to specialized mental health facilities when necessary.

• Implement follow-up measures to determine patient progress, adjusting treatment plans when indicated to ensure optimal recovery.

• Develop relapse prevention plans and strategies that empower patients to maintain stable functioning in their daily lives.

• Identify patients at risk of worsening conditions and work collaboratively to create preventative plans.

• Manage and coordinate the delivery of health services related to behavioral health, ensuring comprehensive care across various treatment providers.

• Document patient progress comprehensively in treatment charts to maintain clear communication with the primary care team.

• Educate patients on self-management skills necessary for active participation in their treatment and recovery process.

• Engage in program evaluation and assist clinic management in assessing behavioral services and outcome metrics.

• Support patients in adhering to medical treatments prescribed by primary care providers, including providing strategies for managing medication side effects.

• Maintain adherence to the Code of Ethical Conduct for Clinical Professionals while fulfilling additional responsibilities as assigned.

Patient Population / Client Focus\n

You will work with a diverse population of clients dealing with various mental health challenges, including anxiety, depression, and other psychosocial issues. Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling or a closely related field is required.

• Possess a current Licensed Professional Counselor (LPC) License, compliant with Texas state regulations.

Compensation and Benefits\n

The salary range for this position is USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
