As part of our online service structure, you will have the autonomy to create your schedule, making it easier to balance professional responsibilities with personal commitments. Key Responsibilities\n

• Conduct comprehensive assessments and ongoing evaluations to tailor mental health support to each patient’s unique needs.
\n
• Facilitate communication among supervisors, direct care teams, and family members to ensure cohesive support for each client’s progress.
\n
• Develop and manage a tentative schedule for patient appointments and necessary home visits, demonstrating a proactive approach to care.
\n
• Coordinate in-home visits that include client observations, chart reviews, staff interviews, and essential data analysis to inform treatment strategies.
\n
• Implement and oversee the effectiveness of individualized treatment plans, ensuring adherence to best practices in mental health care.
\n
• Participate in interdisciplinary team meetings to collaboratively discuss client progress and treatment adjustments, sharing insights and recommendations.
\n
• Compile and submit monthly and quarterly progress notes that reflect each client's development and needs effectively.
\n
• Lead training initiatives to enhance the competencies of staff members in implementing treatment plans and documenting care activities.
\n
• Conduct follow-up interviews and additional home visits as needed to provide ongoing support for clients and their families.

Qualifications\n

• A Master’s or Doctorate degree in a relevant mental health field is required.

• Possession of an active and unrestricted license to practice as an LMFT, LPCC, or LCSW in the state of California.

• A minimum of 2 years of clinical experience in counseling or mental health support is essential.

• A strong understanding of ethical and legal standards related to mental health practice.

• Demonstrated ability to effectively collaborate with a multidisciplinary healthcare team.

Compensation and Benefits\n

• Competitive salary range of $80,000 to $104,000 annually, commensurate with experience and availability of caseload.
