Job Description

Deliver compassionate, skilled nursing care to patients in a field-based environment, supporting individuals and families in their homes during a 13-week full-time contract assignment. This position operates Monday through Friday, 8:00 AM to 5:00 PM, with a caseload expectation of 35 patient visits per day (30 points per week) and requires daily travel within an assigned territory. Mileage reimbursement is provided.

Qualifications:

• Active, unencumbered RN license in the state of practice
• At least 1 year of recent home health experience (field experience strongly preferred)
• Proficient in Home Care Home Base (HCHB) documentation
• Solid OASIS assessment experience
• Reliable transportation, valid drivers license, and auto insurance

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Responsibilities:

• Conduct comprehensive OASIS assessments; develop and implement individualized care plans
• Deliver skilled nursing care: wound care, IV therapy, medication administration, and chronic disease management
• Monitor and report patient progress and changes in condition to the care team and physicians
• Educate patients and family members on treatment plans, medications, disease processes, and preventive care
• Accurately document visits and interventions using HCHB
• Collaborate with physical and occupational therapists, medical social workers, and home health aides for seamless patient care
• Participate in case conferences, team meetings, and ongoing education
• Ensure compliance with regulatory and agency standards for care delivery and documentation

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
