RN - Home Health Profession: Nursing Specialty: Home Health Duration: 13 Weeks Shift: Day Hours per Shift: 8 Hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience required. License: Must have a valid nursing license in hand. Certifications: CPR and BLS required. Must-Have: Proficient in HomeCare HomeBase and Oasis; must have used these within the last 6 months or complete retraining. Description: Provides, coordinates, and directs the provision of home nursing care according to physician's orders. Care is delivered based on agency policies and procedures through the competent application of the nursing process. Responsible to Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits will be required, with periodic office meetings. Must have reliable transportation, a current driver’s license, and agency-required liability insurance. The average number of patients seen per day is 5-8. Contractor will be assigned between 5-10 patients daily, equating to roughly 6-8 points. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Must be willing to travel up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. Local candidates are accepted at the same rate. Must be open to traveling and floating to another clinic if patient census is low. All documentation must be completed within 24 hours of the appointment. Delayed and incomplete documentation that impacts patient care may lead to cancellation. Clinicians are expected to accept all patients scheduled, regardless of distance from the previous appointment. No guaranteed hours are associated with this contract, and clinicians must achieve weekly productivity points of 30. All overtime must be pre-approved. #J-18808-Ljbffr Cynet HealthStaff
