Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews, observational techniques, and review of existing medical records to identify factors impacting patient recovery and rehabilitation.

• Provide empathetic counseling and support to patients and their families, focusing on emotional well-being and adjustment challenges often associated with rehabilitation.

• Collaborate closely with a diverse rehabilitation team to develop individualized treatment plans that address both the psychosocial needs and rehabilitation goals of each patient.

• Assist patients in navigating community resources such as support groups, vocational rehabilitation, and housing assistance to facilitate comprehensive recovery.

• Educate patients and their families about available community resources, as well as post-rehabilitation services to enhance their long-term adjustment and success.

• Engage actively in discharge planning, ensuring smooth transitions from the treatment facility to home or other care settings, thereby maintaining continuity of care.

Qualifications\n

Compensation and Benefits\n

The annual salary for this position ranges from $75,000 to $100,000, commensurate with experience and qualifications.
