Key Responsibilities\n

• Conduct comprehensive patient intakes by focusing on diagnostic and functional evaluations, and subsequently making informed treatment recommendations to the primary care provider.

• Provide brief, targeted therapeutic interventions designed to address the mental health needs of patients, incorporating evidence-based methodologies and personalized care.

• Serve as an integral member of the primary care team, assisting providers in recognizing, treating, and managing mental health and psychosocial issues among patients.
\n
• Initiate follow-up with patients to monitor progress and assess the efficacy of treatment plans, making necessary adjustments based on their ongoing needs.
\n
• Develop individualized relapse prevention plans that support patients in maintaining stable functioning and improving overall mental well-being.
\n
• Document patient progress accurately in the treatment chart, ensuring compliance with all organizational and Federally Qualified Health Center (FQHC) guidelines.
\n
• Educate patients on self-management skills and coping strategies, empowering them to take active roles in their treatment and recovery.

• Work collaboratively with primary care providers to ensure holistic treatment plans that address physical and mental health concerns simultaneously.

• Participate in program evaluation discussions and offer insights into the development of behavioral health services to enhance overall clinic effectiveness.

• Adhere strictly to the ethical standards outlined by the profession, ensuring respect, confidentiality, and positive engagement with all patients.

• Comply with any other duties as assigned to further the mission of the clinic.

Patient Population / Client Focus\n

Your work as a Licensed Professional Counselor will primarily focus on a diverse patient population facing various mental health challenges. Qualifications\n

• A Master’s Degree in Clinical Counseling or a related field is required.

• Current LPC license in the state of Texas.

• Knowledge of developmental psychopathology and family therapy systems.
\n
