Responsibilities
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and interdisciplinary team.
- Contribute to development of an evidence-based, age-appropriate care plan; involve resident/family in goal setting and care decisions.
- Provide direct and indirect nursing care under the supervision of a Registered Nurse; execute nursing interventions as prescribed.
- Monitor resident conditions, recognize changes, and escalate as needed; participate in rounds and care plan reviews.
- Collaborate with physicians, other nurses, and ancillary staff to ensure safe, effective, and high-quality resident care.
- Maintain current nursing knowledge; participate in mentoring and orientation of staff; uphold safety, cost, and quality considerations in care delivery.

Requirements
- Active New York Licensed Practical Nurse (LPN) license
- BLS (American Heart Association or RQI) certification
- Minimum 1 year of LPN experience, preferably in a long-term care setting
- Completion of an LPN education program and licensure documentation
- Ability to work in a long-term care environment with adult and geriatric residents
- Availability to work evenings and required extended weekend/holiday assignments (EOW/Holiday)
