Work Environment\n

This position operates in a remote setting, allowing for flexibility while ensuring access to the necessary tools to effectively serve clients. Despite being remote, you will remain an integral part of our community health clinic, collaborating closely with interdisciplinary teams including healthcare professionals, nurses, and caregivers. Our work hours are primarily Monday through Friday, 8:00 AM to 4:30 PM, though additional evening or weekend hours may occasionally be required based on client needs.

Key Responsibilities\n

• Conduct comprehensive assessments focused on the psychosocial needs of seniors, incorporating emotional, mental, and social factors that contribute to their overall well-being.

• Develop and implement personalized care plans in collaboration with clients and their families that are tailored to specific goals, preferences, and challenges.

• Provide ongoing counseling, emotional support, and guidance to seniors and their families during life transitions, helping them navigate challenges related to aging.

• Advocate for client rights by ensuring access to community resources and services, thereby facilitating continuity of care.

• Collaborate with interdisciplinary teams to enhance care coordination, fostering an environment of shared knowledge and resource utilization.

• Perform regular virtual or in-field visits to monitor clients’ well-being, ensuring they are safe and supported in their living conditions.

• Facilitate communication between family members involved in client care, resolving disputes and encouraging cooperative decision-making.

• Educate clients and their families about available community support programs, resources, and engagement opportunities to enhance their quality of life.

• Maintain meticulous records including client assessments, care plans, and progress notes in compliance with regulatory and ethical standards.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW) in Pennsylvania.

Compensation and Benefits\n

The annual salary for this position ranges from $60,000 to $100,000, commensurate with experience and qualifications.
