Job Summary

We are seeking a highly skilled and compassionate Licensed Physical Therapist (PT) to join our Mobile Outpatient team in Salt Lake City, Utah. As a PT, you will play a vital role in delivering high-quality, patient-centered care to individuals in their own homes and communities. Responsibilities

Conduct comprehensive evaluations and develop individualized treatment plans for patients with various physical therapy needs
Deliver high-quality, patient-centered care in a mobile outpatient setting, working closely with patients and families to achieve their goals
Implement and supervise treatment plans, ensuring effective and efficient patient care
Collaborate with multidisciplinary teams to develop and implement comprehensive care plans
Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
Maintain accurate and detailed records of patient treatment and progress

Requirements

Current licensure as a Physical Therapist in the state of Utah
Master's or Doctoral degree in Physical Therapy from an accredited program
Experience working in a mobile outpatient setting preferred
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Familiarity with electronic medical records (EMRs) and practice management software
