Peer Navigator, Street Medicine (West Hollywood/Marina Del Rey)

The Peer Navigator supports members who are experiencing or are at risk of experiencing homelessness, helping them manage their own health in the West Hollywood/Marina Del Rey, CA area. Responsibilities Build rapport with members by communicating personal experiences to increase the likelihood of positive behavior changes. Coach members to create and implement strategies that minimize risks associated with common health conditions and behaviors. Connect members to appropriate programs to address barriers to care and enhance compliance. Link members to local, county, and state services/resources. Follow up with members and serve as an advocate, delivering medications and other supplies as needed. Coordinate and/or accompany members to medical and social services appointments that fall outside of the street medicine unit. Introduce tools to promote self‑management and self‑efficacy. Complete all required organization and health‑plan specific documentation in a timely fashion, including entering all patient touch points and encounter notes, updating member profiles, and submitting monthly audit checklists and individual care plans. May occasionally drive the medically equipped van. Schedule encounters based on member acuity using the Healthcare in Action electronic health record system. Work under the supervision of a Social Worker to ensure proper escalation of cases and coordination of care. Actively support the achievement of HIA’s vision and goals, particularly housing placement for unsheltered members. Qualifications GED or high school diploma required, plus 3+ years of relevant work experience; or bachelor’s degree preferred, plus 1‑2 years of relevant experience. Direct work experience, lived experience, training, or volunteer work with patients experiencing homelessness and those with active mental health and substance use conditions. Relevant experience in similar community settings (e.g., working with disadvantaged groups, outreach). Ability to work in a dynamic, outdoor environment and independently as part of a team. Strong organizational, prioritization, and communication skills, including the ability to articulate personal experiences. Excellent oral communication and interpersonal skills. Reliable transportation and proficiency in operating a computer and multiple software systems (e.g., HMIS/Coordinated Entry System). Must have a valid driver’s license, insurance, and a clean driving record. Will travel 50% or more of the time. Knowledge of the healthcare system, Medicare and Medicaid, and mobile clinic operations is an advantage. Benefits Base salary range: $21.78 to $31.15 per hour. Annual employee bonus program.
