Peer Navigator, Street Medicine (Anaheim, CA)

Role Overview Healthcare in Action delivers street medicine by using a medically equipped van to provide care to people experiencing homelessness. The Peer Navigator supports members at risk of homelessness in the Anaheim, CA area by helping them manage their health, motivating behavioral change, providing education, peer support, housing and resource navigation, and sharing personal experiences. Responsibilities Build rapport with members by sharing personal experiences to encourage positive behavior changes. Coach members to develop and implement strategies that reduce risks associated with common health conditions and behaviors. Connect members to programs that remove barriers to care and improve compliance. Link members to local, county, and state services and resources. Follow up with members and serve as advocate, delivering necessary medications and supplies. Conduct regular outreach to onsite patients at the Nav Center and to offsite patients as needed. Provide accompaniment and support for medical, dental, and other appointments. Collaborate actively with staff of the Navigation Center and with HIA team members for patient care management. Introduce tools to promote self‑management and self‑efficacy. Complete required organization and health‑plan documentation in a timely fashion, including electronic health records, risk assessments, and monthly audit checklists. Schedule encounters based on member acuity using HIA’s electronic health record system. Work under the supervision of a Social Worker to ensure proper escalation and coordination of cases. Support the achievement of HIA’s Vision and Goals, particularly housing placement for unsheltered members. Qualifications G.E.D./High School diploma required; Bachelor’s degree preferred. 3+ years of experience working with individuals experiencing homelessness, including those with mental health or substance use conditions; or related community outreach experience. Direct or lived experience with homelessness, mental health, and substance use conditions. Ability to work outdoors in a dynamic environment, independently and as part of a team. Strong organizational, prioritization, and communication skills. Reliable transportation, valid driver’s license, clean driving record, and automobile insurance. Proficiency in using computers and multiple software systems, including electronic health records, time‑and‑attendance tools, and call‑center software. Knowledge of HMIS/Coordinated Entry System preferred. Travel 50% or more of the time.
