Roles & Responsibilities

• Provide compassionate physical therapy services to patients in their own homes
• Develop and update individualized care plans in collaboration with physicians
• Conduct comprehensive evaluations and reassessments to track progress
• Educate patients and caregivers on home exercise programs and adaptive strategies
• Supervise and mentor Physical Therapist Assistants and Home Health Aides
• Adhere to Medicare and state regulations, including OASIS and ICD-10 documentation
• Participate in quality improvement activities to drive clinical excellence
• Work autonomously while maintaining excellent communication with the care team

Qualifications

• Graduate of an accredited physical therapy program (DPT or equivalent)
• Active Physical Therapist (PT) license in the state of New Mexico
• Current CPR certification to keep you and your patients safe
• At least 2 years of clinical experience (home health experience is a plus, but we'll train the right fit)
• Reliable transportation, a valid driver's license, and auto insurance for making home visits

Education

• Graduate of an accredited physical therapist curriculum (CAPTE-approved or equivalent)

Knowledge & Skills

• Clinical expertise in physical therapy, including patient evaluation and treatment planning
• Familiarity with OASIS documentation and ICD-10 coding (we'll help you brush up if needed)
• Understanding of HIPAA, infection control, and safety protocols
• Comfort with EMR systems and digital documentation
• Strong interpersonal skills to connect with patients of all backgrounds
• Ability to work independently and adapt to ever-changing home environments
