Responsibilities The Physical Therapist will primarily provide outpatient rehabilitation services in an in‑home setting for the older adult population. The setting may be a patient’s private residence or room in an independent living facility. Powerback Rehabilitation To You allows evaluation and treatment in the most functional environment – the patient’s home. The Physical Therapist assesses needs, develops and delivers physical therapy programs, provides referrals to other disciplines, implements individualized therapy to restore, reinforce, and enhance performance, instructs and trains patients and caregivers, and promotes patient independence. Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards. Ensure adherence to department and patient treatment schedules, timely clinical and billing documentation, and participation in work‑related meetings. Attend and contribute to patient care, staffing conferences, and related meetings. Make referrals to other disciplines and agencies to address comprehensive patient needs. Consult other rehabilitation team members to select the most appropriate therapeutic regimen. Adapt program and/or plan of care according to the needs of the individual patient. Promote a safe environment and enforce elimination of fire and safety hazards. Order supplies and equipment as necessary. Clean, maintain, identify, and report need for repair of equipment as necessary. Put patient service first: ensure patients and families receive the highest quality service in a caring, compassionate atmosphere that recognizes individual needs and rights. Screen, examine, and evaluate patients, including history, system review, and application of appropriate tests and measures; synthesize information, interpret findings, establish a diagnosis, identify impairments, determine predicted improvement and time required; establish precautions/contraindications and design and plan of care. Develop appropriate treatment goals and methods in collaboration with the patient and caregivers; implement the PT treatment plan; complete all related documentation and record keeping. Services include but are not limited to: Therapeutic exercise (aerobic conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility exercises, gait and locomotion training, neuromotor development training, relaxation, strength, power, and endurance training) Functional training Manual therapy Airway clearance techniques Integumentary repair and protection Use of electrotherapeutic, physical agent, and mechanical modalities Health and wellness Education on chronic disease management Falls assessment and interventions to reduce fall risk Case management in the home and community Design, fabricate, fit, and/or apply adaptive devices, and train patients in their use. Select appropriate activities suited to the individual's physical and mental capacity, intelligence level, and interest. Instruct patients in mobility skills and evaluate progress, attitudes, and behavior related to rehabilitative potential. Perform routine reexamination as needed to modify/progress treatment plan. Incorporate health and wellness programming into treatment plans. Perform home and community mobility assessments and provide recommendations for safe mobility. Perform other related duties as required. Qualifications Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility. Master’s degree in Physical Therapy; or Master’s degree in a Physical Therapy related field plus a Bachelor’s degree in Physical Therapy; or Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience. Physical Therapy degree earned outside the U.S. must be evaluated as U.S. equivalent. Valid driver’s license or reliable independent transportation to patients’ residences.
