Experience: One (1) year of previous long-term care experience

License: NYS Nursing Assistant Certification

Certifications: BLS (AHA or RQI only)

Must-Have: High School Diploma or GED

Description: Meet residents' Activities of Daily Living (ADL) needs as outlined in the plan of care. Ensure a safe and comfortable environment in accordance with policies and procedures. Assist residents with hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting, turning/positioning, and skin care. Complete documentation per protocol. Maintain resident safety and environment as outlined in the plan of care. Answer call lights and alarms promptly. Provide safety mats and low beds as needed. Utilize restraints under the direction of Nursing. Maintain cleanliness of the unit and adhere to behavior and fall prevention protocols. Monitor for malfunctioning equipment and ensure its proper use. Document all procedures according to protocol. Meet residents' nutritional needs as outlined in the plan of care. Assist with meal delivery, tray preparation, and feeding as required. Honor residents' preferences and choices. Meet residents' rehabilitative needs in accordance with the plan of care. Assist with range of motion, ambulation, transfers, and proper body alignment. Utilize assistive devices as needed and document accordingly. Report residents' rights, refusals, and concerns to the Charge Nurse in a timely manner. Identify and report changes in residents' physical, emotional, and psychosocial status to the Charge Nurse promptly. Be aware of potential occupational exposure to bloodborne pathogens. Tasks may involve contact with blood, body fluids, or tissues.
