Role Description The Family Services Coordinator is a full-time remote role responsible for supporting families through assessment, planning, and ongoing case coordination. In this position, the coordinator conducts intake interviews, evaluates family needs, and develops individualized service plans in partnership with clients. The role involves connecting families with appropriate community resources, social service agencies, and internal programs, while monitoring progress and adjusting plans as needed. The coordinator maintains accurate case records, prepares reports, and communicates regularly with families, providers, and colleagues to ensure continuity of care. This role also supports crisis intervention, advocates for client needs, and participates in virtual team meetings, trainings, and quality improvement initiatives.\n
Qualifications

• Demonstrated experience in Social Work or Family Services, with a strong commitment to supporting diverse families.

• Proficiency in Case Management, including assessment, service planning, referral coordination, and follow-up.

• Strong Communication skills, including clear written documentation and effective, empathetic verbal communication in virtual settings.
\n
• Understanding of Social Services systems, community resources, and eligibility processes for public and nonprofit programs.
\n
• Ability to work independently in a remote environment, manage multiple cases, and prioritize tasks effectively.
\n
• Experience with electronic case management or client tracking systems and basic proficiency with standard office software.
\n
• Relevant education such as a degree in Social Work, Human Services, Psychology, or a related field; equivalent experience considered.
\n
• Knowledge of trauma-informed and culturally responsive practices; multilingual skills are a plus.
