Position Overview

Touchstone Wellness & Physical Therapy Clinic is seeking a dedicated Full-Time Licensed Physical Therapist to join our team in Winter Haven, Florida. Key Responsibilities

• Conduct comprehensive patient evaluations by reviewing medical history, performing specific assessments, and developing tailored treatment plans aimed at optimizing recovery and enhancing mobility.
• Clearly articulate evaluation findings and proposed treatment strategies to patients and their families, involving them in setting achievable goals and expectations.
• Implement evidence-based treatment protocols that evolve with both the patient's condition and advancements in healthcare practices, ensuring an efficient path to recovery.
• Collaborate with a multidisciplinary team, recognizing and documenting any changes in a patient's condition that may necessitate modifications in care plans or referrals for additional medical intervention.
• Facilitate discharge planning and post-treatment follow-up, ensuring that all documentation is accurate, timely, and reflective of the patient's rehabilitation journey, including detailed progress reports and evaluations.
• Manage your schedule effectively, achieving productivity metrics while adhering to meeting deadlines and participation in clinic marketing efforts to promote services within the community.
• Contribute to departmental objectives and ensure compliance with all relevant regulations and accreditation standards, demonstrating accountability and professionalism in all interactions.
• Pursue ongoing professional education and development opportunities, showcasing initiative in your own growth as well as the improvement of the clinic's services.
• Enhance patient access and experience by demonstrating flexibility and adaptability in service delivery, addressing any barriers to care, and engaging in quality improvement initiatives.

Patient Population / Client Focus

Our patients range from individuals recovering from surgery to those dealing with chronic conditions, sports injuries, or neurological disorders. You will play a critical role in helping these diverse patient populations regain their independence and improve their quality of life through tailored care and support.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• Current state licensure as mandated for physical therapy practice in Florida.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• Strong communication skills and a collaborative spirit, enabling effective interaction with patients, families, and colleagues.

Preferred Experience

• Previous experience in an outpatient rehabilitation setting is a plus but not required.
• Familiarity with electronic health record (EHR) systems and clinical documentation processes.
• Any specialized training or certification in specific therapeutic areas will be advantageous.

Compensation and Benefits
