Registered Nurse - Utilization Review / CDI

Job Description

Job Summary:

The Registered Nurse Non-Certified Case Manager is responsible for coordinating and managing patient care plans, working closely with physicians and multidisciplinary teams to ensure optimal health outcomes. The role involves assessing patient needs, developing care strategies, and monitoring progress to facilitate effective discharge planning and service integration.

Responsibilities:

• Conduct comprehensive assessments of patients within regulated timelines to identify suitable candidates for case management.
• Develop and implement individualized care plans in collaboration with patients, caregivers, physicians, and other healthcare professionals.
• Monitor patient progress continuously, evaluate care plan effectiveness, and adjust interventions as needed.
• Maintain an active caseload, providing regular outreach and ongoing management of patient cases.
• Promote seamless integration of behavioral health, long-term services, and community supports to enhance continuity of care.
• Uphold compliance with education and training requirements, including DNV standards and organizational policies.

Qualifications:

• Hold Montana Registered Nurse licensure and valid American Heart Association BLS certification.
• Possess a minimum of 2 years of nursing experience, including utilization review expertise.
• Demonstrate strong clinical judgment, patience, and professionalism in high-pressure environments.
• Exhibit excellent verbal and written communication skills with tact and discretion.
• Proficiency in electronic medical records, MS Office, and internet applications.
• Preferably, hold a Bachelor's Degree in Nursing with experience in acute hospital settings as a case manager.
