Key Responsibilities\n

• Conduct individual and group therapy sessions, utilizing evidence-based techniques to assist clients in processing their emotions and experiences associated with hospice care.

• Support patients with a broad spectrum of mental health challenges, conducting thorough assessments to tailor interventions that best meet their needs.

• Accurately document patient progress and treatment plans in our electronic health record system, ensuring compliance with state and federal regulations.

• Drive case collaboration with multidisciplinary team members, including physicians, nurses, and caregivers, to formulate effective care plans that address both physical and emotional needs.

• Maintain clinical documentation in a timely manner to support treatment continuity and ensure clear communication among team members.

Patient Population / Client Focus\n

The LCSW will primarily work with patients who are receiving hospice care, which often involves addressing complex emotional and psychological needs. This role requires sensitivity to the unique circumstances of terminal illnesses, focusing not just on the patients but also on aiding family members as they cope with loss and grief. You will also engage with diverse populations, recognizing various cultural and personal beliefs regarding end-of-life care.

Qualifications\n

• A minimum of a Master’s degree in Social Work or a closely-related field.

• Possession of an active and current LCSW license in the state of Pennsylvania.

• At least two years of experience working in a mental health environment, ideally in outpatient settings.

• Demonstrated proficiency in medical documentation processes and record-keeping practices.

Preferred Experience\n

• Expertise in crisis intervention, enabling you to respond effectively to patients experiencing acute distress.

• Experience working with children and/or adolescents, especially in a hospice or palliative care context, is a plus.

• Familiarity with motivational interviewing techniques to enhance client engagement and commitment to treatment.

• Knowledge of gender-specific issues in counseling also preferred, to better support diverse client needs.

Compensation and Benefits\n
