Key Responsibilities\n

• Directly engage with clients in various capacities, including teletherapy, to assess and provide treatment for mental health conditions related to their hospice care.

• Conduct comprehensive assessments to support the development of individualized care plans that address clients’ emotional and psychological needs.

• Collaborate with interdisciplinary teams to create and implement integrated care plans, facilitating communication among clients, families, and healthcare providers.

• Identify and intervene in crises, offering timely support and resources to clients facing acute emotional distress.
\n
• Maintain a caseload that includes individual, group, and family therapy, ensuring that treatment goals are tailored to each client's presenting issues and requests.

• Ensure thorough documentation of client interactions, treatment plans, and progress reports according to organizational standards, contributing to ongoing quality improvement initiatives.

• Monitor clients' statuses, including admissions, ongoing assessments, and discharge criteria, while being proactive in service delivery.

• Participate in case consultations, team meetings, and professional development workshops to continually enhance your clinical skills and knowledge.

• Provide timely on-call support during evenings and weekends as assigned, addressing client needs swiftly and compassionately.

This role primarily focuses on clients receiving hospice care, many of whom may be experiencing significant mental health challenges due to grief, anxiety, and depression stemming from terminal illness. Qualifications\n

• A Bachelor’s degree in Social Work or a related field is required.

Compensation and Benefits\n

The position offers a competitive salary range of USD 70,000 to 85,000 per year, commensurate with experience and qualifications.
