Key Responsibilities\n

• Plan, coordinate, and implement individualized support packages for clients facing various emotional and psychological challenges, including those related to end-of-life issues.

• Conduct thorough assessments of clients’ conditions by interviewing service users to identify their needs, strengths, and any weaknesses, applying a client-centered approach.

• Develop tailored, measurable goals for each case and continually monitor client progress to adjust treatment plans as needed.

• Provide information and counseling during sessions to guide clients towards effective coping strategies and positive decision-making.

• Maintain meticulous records of client interactions and progress, ensuring compliance with relevant documentation standards.

• Represent our clients' interests by participating in case conferences and providing evidence in court when necessary.

• Act as a key worker and engage with multidisciplinary teams, fostering collaboration to achieve the best outcomes for clients.

• Facilitate referrals to community services, ensuring clients receive comprehensive resources that support their recovery and well-being.

Patient Population / Client Focus\n

The primary focus of this role involves serving a diverse population of clients who may be experiencing significant life transitions, including terminal illnesses, grief, and loss. You will work closely with patients and their families to navigate the emotional complexities of hospice care, ensuring they receive empathetic support and practical resources in a timely manner.

Qualifications\n

• Master’s degree in Social Work (MSW) accredited by the Council on Social Work Education or a comparable degree in psychology or sociology.

• Current state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
