The mission of Puyallup Tribe’s Wrap Around Department is to strengthen families and communities by empowering youth to resolve conflicts and overcome obstacles in order to become successful students and contributing adults. Juvenile Healing Wellness Program (JHW) is a restorative justice strategy funded by OJJDP (Office of Juvenile Justice Delinquency and Prevention) intended to redirect youth into pathways of Puyallup culture and history while supporting youth in “pro‑social” productive settings. Puyallup Tribe’s “Youth Services Division” operates a variety of innovative programs proven to increase student performance, reduce school dropout rates, gang involvement, youth violence and crime and ultimately help preserve families. Networking between strategies is essential for Puyallup Tribal youth to be successful and productive members of the tribe and community. The Case Manager role will be to appropriately manage assigned youth to obtain required support and assistance that are referred to the JHWC. Continued case support and monitoring of youth would also be a requirement in this position. Essential Duties and Responsibilities Under the direction and supervision of the Wrap Around Director, the Case Manager will be responsible for managing the program referrals and helping manage intake coordination. Providing resource navigation and linkages to referred youth and their families. Assisting in program referral/enrollment for clients. Consistent and regular contact with families and providers to monitor progress. Providing comprehensive strengths‑based intake to referred youth who are either involved or at risk of being involved in the juvenile justice system. Triaging referred youth to the appropriate internal Puyallup Tribal program(s). Case Manager Specific Duties Refer participants to service providers for social and developmental services such as victim/offender and family mediation, mental health treatment and/or counseling, education rights assistance, academic assistance, housing assistance, life skills, mentorship, cultural/ceremonial, educational, and vocational/job training. Maintain accurate case files; collect and report data as needed for reports and evaluation. Call referred clients to determine eligibility for program and willingness to enroll. Maintain regular contact with providers and clients to monitor progress. Assist with the development of effective assessment and case management procedures and systems. Make home and school visitations as needed. Provide timely follow up with regard to referrals via email, phone, etc. to build rapport and identify appropriate services needed. Review incoming referrals, determine eligibility, and triage referrals to the appropriate Puyallup Tribal program. Conduct pre‑screening assessments with youth and families to determine appropriate service placement. Work closely with Puyallup Tribe’s Wrap Around Department and referring agencies regarding referral processes, enrollment outcomes and service needs of referred youth. Referring agencies include police departments, schools, probation department and courts. Process referrals accurately and efficiently within established productivity guidelines and time frames. Maintain accurate case files; collect and report data as needed for reports and evaluation. Act as a preceptor to assigned office interns. Participate in staff training and partner meetings. Performs other duties as assigned. Supervisory Responsibilities Acts as a preceptor to assigned office interns, and manages assigned youth. Qualifications and Requirements Education and/or Experience Requires a Bachelor’s Degree in social work and/or psychology AND one year of case management experience in social work and/or juvenile justice or a closely related field or the equivalent combination of education/experience. Familiarity with the Wrap Around Case management approach preferred. Language Skills Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers and the general public. Mathematical Skills Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry. Reasoning Ability Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Certificates, Licenses, and Registrations Must have and maintain a valid and unrestricted Washington State driver’s license and proof of insurance. The Driver’s License must not contain any restrictions that would otherwise prevent an employee from complying with the Tribe’s Vehicle Use Policy or operating a GSA vehicle. Other Requirements Ability to maintain the strictest confidentiality; Familiarity and working knowledge of Native American issues and tribal governments; Demonstrated excellence in verbal and written communication skills; Possesses outgoing nature and enthusiasm for the Tribe’s mission; Can work well under pressure independently and meet deadlines; Ability to work cooperatively and positively with many people, including healing‑to‑wellness clients, court staff, healing‑to‑wellness stakeholders, program managers and the grants team. Requires occasional travel using employee’s own transportation. Inside majority of time with limited exposure to outside elements. Regularly drive back and forth to off‑site locations.
