Moms In Motion - A Virginia Service Facilitation Provider

Moms In Motion in Virginia is seeking a Service Facilitator (SF) Case Manager to visit clients at home and guide families through Virginia's Medicaid Waiver programs. You’ll write plans of care, coordinate authorizations, and build trusted relationships with families in the community. Must live in VA with at least 2 years supporting individuals with disabilities or the elderly.
