Duties include assessment of patients, developing plan of care, implementing the plan of care and evaluating plan of care. Position requires a high quality of care in order to meet the Hospital's core mission. Qualifications Associate Degree in Nursing from a state-accredited school (Required) Bachelor's Degree in Nursing (Preferred) Work Experience: 1 or more years of Nursing Experience (Preferred) Licenses and Certifications: Registered Nurse (RN) with current Georgia license, Basic Life Support (BLS). If working on a designated stroke unit, Advanced Stroke Life Support (ASLS), and National Institutes of Health Stroke Scale (NIHSS) must be obtained within 2 years of hire. Essential Functions Collect data systematically, identify issues and develop diagnoses based on assessment data. Prioritize data collection according to patient condition and needs. Identify expected outcomes and plan based on diagnoses or issues. Develop an individualized plan with strategies addressing diagnoses. Provide continuity through a plan that directs other team members. Implement the plan promptly and safely, documenting changes or omissions. Consult with healthcare providers and make referrals as appropriate. Utilize community resources and systems to implement the plan. Submit required information and data in a timely manner. Document processes accurately, using correct terminology and required style. Develop expected outcomes that provide direction for continuity of care and involve patient/family and other disciplines. Evaluate plan in relation to patient responses and expected outcomes. Document evaluation results and modify outcomes based on assessment changes. Apply results from quality improvement activities to nursing practice. Use creativity and evidence‑based knowledge for practice improvement. Participate in quality improvement activities. Take accountability for individual practice and delegation to ensure optimum care. Practice compassion, respect, and patient advocacy. Ensure patient safety through competence and growth of self and others. Contribute to a healthy work environment through shared decision‑making. Collaborate with other health professionals to meet healthcare needs. Coordinate care across settings among caregivers. Engage in teamwork as a team player and builder.
