Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health to support patients in Havertown, PA and surrounding areas. Responsibilities Home Health registered nurse case managers lead and collaborate to provide management and delivery of patient care plans, ensuring quality patient care in a team‑based environment. Qualifications RN license in the state of practice. Graduate from an approved school of practical nursing. At least one year of RN experience and clinical competence to deliver patient care. Current driver’s license and ability to spend approximately 20% of the day driving to patient locations. Commitment to consistently meet critical charting deadlines.
