Community Case Manager: Mental Health & Independent Living
ICL
ICL is seeking a dedicated professional to support individuals with mental disabilities in their independent living in Brooklyn, NY. Under supervision, you will assist clients in accessing medical, social, and vocational services and provide crisis intervention as needed. You will travel to consumers' residences, develop individualized service plans, and document progress while advocating for client needs.
