Responsibilities
- Perform initial PT evaluation and develop an individualized plan of care for home health patients within the care framework.
- Implement therapeutic interventions including therapeutic exercise, gait and balance training, ROM, strength training, and functional mobility training.
- Educate patients and families on home exercise programs, safety, and strategies to support daily activities and functional independence.
- Monitor and document patient progress, adjust treatment plans, and communicate updates to the supervising clinician and care team.
- Collaborate with interdisciplinary team to coordinate care, schedule visits, and support discharge planning and community resources.
- Ensure accurate, timely documentation in the patient record and maintain compliance with applicable standards and regulations.

Requirements
- Licensure: Active physical therapy license in the state of practice (California)
- Certifications: CPR certification or Basic Life Support (BLS or equivalent) required
- Experience: Minimum 2 years of physical therapy practice
- Specialty: Focused experience in Home Health or equivalent care setting
- Ability to work in a home health care delivery model and travel as assigned
- Strong documentation, communication, and patient education skills
