Case Manager - Young Children Ages 3-6

Job Responsibilities Under the general direction of a Case Management Supervisor, responsible for the full range of case management duties, including cases requiring intensive case management and high levels of expertise, and implementing the clients' service plans. If assigned to support young children (ages 3-6), will assist Intake with determining Lanterman eligibility for children under "Provisional Eligibility" criteria prior to turning 5 years old. Minimum Education and Experience Requirements A Master's degree in a field related to developmental disabilities. Or Bachelor's degree in a field that offers content related to developmental disabilities, such as social work, psychology, counseling, special education, or nursing, and three years of employment in a field related to developmental disabilities.
