Work Environment

This position is centered in a home care setting, requiring the Physical Therapist to travel to patients' homes and provide one-on-one therapy sessions. Our team fosters an environment of support and collaboration, ensuring that all staff members can contribute to the success of patient treatments. The schedule is Monday through Friday from 9 AM to 5 PM, allowing for a balanced work-life integration.

Key Responsibilities

• Conduct initial and follow-up physical therapy evaluations in accordance with established standards of practice and state regulations.
• Assess patients' mobility, strength, and functional abilities to create achievable and realistic rehabilitative goals tailored to individual needs.
• Develop and implement comprehensive Plans of Treatment based on thorough evaluations, incorporating input from patients, caregivers, and interdisciplinary team members.
• Regularly monitor and adjust treatment plans based on patient progress, ensuring that therapy techniques remain effective and responsive to changing needs.
• Document all patient interactions and treatment strategies according to facility protocols and compliance standards, maintaining clear and organized patient records.
• Enhance professional knowledge by participating in ongoing educational opportunities, such as workshops and training sessions, and by reviewing the latest research in the field.
• Collaborate with fellow team members to maintain a clean, organized, and efficient therapy environment that supports optimal patient care.

Patient Population / Client Focus

This role encompasses treating a diverse patient population, including individuals recovering from orthopedic injuries, post-operative rehabilitation, and chronic pain management. Our therapists tailor their approaches to patients of all ages, ensuring that care is personalized and respectfully aligns with each patient's recovery journey. Demonstrating empathy and effective motivational techniques is paramount, as our therapists guide patients through challenging but rewarding rehabilitation processes.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program is required; a Master's or Doctoral degree is preferred.
• Possession of an active Physical Therapy license in the state of Virginia is essential.
• Current certification in Basic Life Support (BLS) with AED is mandatory.
• At least 2 years of hands-on experience in physical therapy is required, demonstrating a proven track record of treating patients effectively.
• A compassionate demeanor, strong interpersonal skills, and the ability to inspire patients to excel in their recovery efforts are essential attributes.
• Thorough understanding of rehabilitation equipment, its safe usage, and best practices for preventive maintenance.

Preferred Experience

Experience in home care settings and familiarity with outpatient orthopedic rehabilitation practices are highly valued, as is an eagerness to continually learn about new therapeutic techniques and interventions to enhance patient care.
