Wapakoneta Health Care Center is now under new management , and exciting changes are underway! We are building a strong, resident-focused team centered on compassion, accountability, teamwork, and clinical excellence.

We are currently seeking an experienced and motivated Resident Care Coordinator (LPN) to join our growing team.

The Resident Care Coordinator plays a key leadership role in supporting the daily clinical operations of the facility and helping ensure our residents receive exceptional, compassionate care. This position works closely with nursing leadership, frontline caregivers, residents, and families to promote strong communication, consistent care delivery, and a positive resident experience.

The ideal candidate is a Licensed Practical Nurse (LPN) who is organized, dependable, approachable, and passionate about supporting both residents and the nursing team. Successful team members at Wapakoneta Health Care Center demonstrate work ethic, integrity, competence, and compassion in everything they do.

Job Responsibilities and Duties:

• Provide daily care to residents

• Assist with dining needs

• Respond promptly to call lights

• Chart and document resident care activities

• Engage, converse, and interact with residents to promote a positive environment

• Lead and coordinate resident care with a rehabilitation focus

• Provide leadership for staff while supporting high-acuity residents

• Ensure quality management practices are followed

• Perform resident assessments and maintain accurate documentation

• Collaborate with team members to deliver excellent care for elderly residents

Requirements:

• Unencumbered LPN or RN license

• Previous long-term care experience

• Strong leadership skills and commitment to Total Quality Management

• Excellent assessment and documentation skills

• Passion for working with the elderly

• Experience with behavior-challenged residents strongly preferred
