Registered Nurse

Job Summary:


The Registered Nurse in the TCU (Transitional Care Unit) at a skilled nursing facility is responsible for providing comprehensive nursing care focused on rehabilitation and sub-acute care. This role involves managing a patient load of approximately 8 to 10 patients per nurse, delivering skilled nursing services including patient assessments, medication administration, and treatment planning. The nurse collaborates closely with interdisciplinary teams to ensure quality care for patients recovering from various system diagnoses, excluding custodial or long-term care. A minimum of one year of nursing experience and BCLS certification are required.

Responsibilities:

• Provide skilled nursing care to patients in a sub-acute rehabilitation setting.
• Perform patient assessments, administer medications, and develop treatment plans.
• Collaborate with interdisciplinary team members to coordinate patient care.
• Monitor patient progress and document clinical information accurately using Epic charting system.
• Maintain patient safety and comfort while adhering to facility policies and procedures.
• Communicate effectively with patients, families, and healthcare providers.
• Adhere to admission criteria specific to the skilled nursing facility.

Required Skills & Certifications:

• Active Registered Nurse license valid in the state of practice.
• Minimum of 1 year nursing experience, preferably in sub-acute or rehabilitation care.
• Basic Cardiac Life Support (BCLS) certification.
• Proficiency in clinical assessment, medication administration, and treatment planning.
• Ability to manage a nurse-to-patient ratio of approximately 1:8-10.
• Strong communication and interpersonal skills.

Preferred Skills & Certifications:

• Experience working in a skilled nursing facility or transitional care unit.
• Familiarity with Epic electronic medical record system.
• Additional certifications related to rehabilitation or sub-acute nursing care.

Special Considerations:

• Weekend work required every other weekend.
• On-call status is pending and may be required.
• Nurse-to-patient ratio typically ranges from 1:8 to 1:10.
• Floating between similar units is pending and may be necessary.
• All Return to Office (RTO) requests must be submitted with initial application and are subject to facility approval.
• Facility is a sub-acute care environment, not providing custodial or long-term care.

Scheduling:

• Standard shifts with weekend work every other weekend.
• Block scheduling is not accommodated.
• Guaranteed hours with the ability to call off once per two-week pay period.
