Patient Care Coordinator

Patient Care Coordinator Location: 345 E 37th St, New York, NY 10016 Work Setting: Core - On Site Hours: 8:30AM-5:00PM Position Summary This position is responsible for accomplishing various task that contributes to the organization success and patient satisfaction. Team Providers, Medical Assistants, Nurses (RN/LPN), PSA, and echo techs averaging 21 members daily. Fast-paced, diverse team environment with responsibility for day-to-day operations, compliance, and coverage across sites as needed. Primary Responsibilities Obtain and update patient information Perform insurance verification Obtain patient signatures File, Fax and maintain medical records Schedule appointments Answer incoming and outgoing calls Check In and Check Out patients Manage work queues, referrals, and front desk in-basket Schedule specialist appointments and follow-up services Ensure continuity of care through scheduling and tracking systems Scan and assign documents to electronic charts Maintain HIPAA and OSHA compliance Crosstrain and assist at other locations as needed Perform other duties as assigned Qualifications Background: Healthcare Required Experience: 2 yearsMedical, dental, and vision insurance are available to qualified candidates who meet eligibility requirements.
