What You’ll Do

As a Home Health Physical Therapist, you’ll provide individualized, evidence-based rehabilitation to patients in their homes, helping them achieve their highest possible level of function and independence.

• Evaluate patients’ physical abilities, mobility, strength, balance, and functional limitations.

• Develop and implement individualized treatment plans based on patient needs and goals.

• Provide therapeutic interventions to improve mobility, restore function, alleviate pain, and prevent further disability.

• Help patients recover following illness, injury, surgery, or changes in functional status.

• Educate patients and caregivers on therapeutic exercises, safe mobility, fall prevention, and home safety.

• Monitor progress and adjust treatment plans to support measurable clinical outcomes.

• Collaborate with nursing, providers, other rehabilitation professionals, and the interdisciplinary care team.

• Complete timely clinical documentation and communicate changes in patient status.

• Coordinate care with scheduling and clinical leadership to promote an efficient workflow and positive patient experience.

What We’re Looking For

• Current Pennsylvania Physical Therapist license in good standing.

• Degree in Physical Therapy from an accredited program.

• Experience in home health, geriatrics, skilled nursing, long-term care, or a similar patient-centered setting is strongly preferred.

• Strong clinical assessment, treatment planning, and documentation skills.

• Ability to work independently while maintaining close communication with an interdisciplinary team.

• Excellent communication and patient education skills.

• A genuine passion for helping patients maintain independence and improve their quality of life.
