Work Environment

As a Physical Therapist in our Home Care unit, you will work directly in patients' homes, providing therapeutic services in a comfortable, familiar setting. Key Responsibilities

Your primary responsibilities as a Physical Therapist will include:

• Conducting thorough patient assessments to evaluate physical capabilities and limitations.
• Developing and implementing individualized care plans in collaboration with the healthcare team and patients, focusing on recovery goals.
• Administering a variety of therapeutic interventions and modalities to enhance mobility and function, ensuring to adhere to best practices and professional ethics.
• Providing education and training to patients, family members, and caregivers regarding therapeutic exercises, assistive devices, and proper techniques to maintain progress outside of therapy sessions.
• Coordination of daily caseloads, prioritizing patients' needs and managing time effectively to ensure comprehensive care delivery.
• Documenting patient progress and treatment outcomes in accordance with regulatory guidelines, utilizing Electronic Health Records (EHR) systems.

Patient Population / Client Focus

Your role will involve working closely with a diverse demographic, including individuals recovering from surgeries, those managing chronic conditions, and children requiring specialized pediatric therapies. Qualifications

To be considered for this fulfilling role, candidates must meet the following criteria:

• Possession of a Doctor of Physical Therapy (DPT) degree from an accredited program.
• A valid Physical Therapy license in California.
• Basic Cardiac Life Support (BCLS) certification, to be obtained within the first 30 days of employment.

Preferred Experience

While no prior experience is required, applicants with any of the following will have a distinct advantage:
