Job Description

Job Title: RN (Registered Nurse), Home Health

City/State: Bardstown, Kentucky

Shift: 8H Days Monday-Friday 8am-5pm with on-call every 3rd weekend and 1 night per week

Contract Length: 13 weeks

Start Date: 08/24/2026

Job Description:

This position requires an RN with Home Health experience and proficiency in Homecare Homebase (HCHB). The role primarily involves providing care to pediatric and adult patients within the coverage area of Nelson, Spencer, and southern Bullett counties.

Responsibilities:

Provide care primarily to pediatric and adult patients
Schedule and conduct start of care, recertifications, and daily visits
Be on-call one weeknight and every 3rd weekend
Collaborate with Home Health/Hospice Aides

Requirements:

Homecare Home Base experience
Background in Home Health
Experience in hospice care
Valid Driver's License and Insurance
BLS Certification
