Registered Nurse - Patient Progression - P/T with Benefits Evenings

About This Role The Patient Progression RN at Hackensack Meridian Health is responsible for coordinating patient placement and flow to ensure effective and efficient patient care. This role is ideal for those with strong clinical expertise and a commitment to improving healthcare delivery. Highlights Location: Brick, NJ Facility: Hackensack Meridian Health Schedule: Part-time, Evening shift What You’ll Do Coordinate the placement of elective and emergent admissions and transfers. Communicate the Patient Progression plan to staff and physicians. Review patients' conditions for appropriate placement. Track bed availability and maintain logs/records. Conduct walking rounds on nursing units to identify discharge barriers. Act as a resource for patient placement staff regarding unit capabilities. Manage patient transfers from the ED/PACU or direct admits. Provide status reports to the Manager of Patient Progression. Assist with telemetry transports as needed. Escalate barriers to discharge or transfer. Requirements Education Bachelor's degree in Nursing from an accredited nursing program Experience 3 years of professional nursing experience Previous experience in Capacity Management/Patient Progression preferred Licenses & Certifications Current NJ State Professional Registered Nurse License National Certification desirable Telemetry Certification preferred About Hackensack Meridian Health Hackensack Meridian Health is a leading healthcare provider committed to delivering exceptional patient care.
