Work Environment

Key Responsibilities

• Conduct comprehensive evaluations to assess the needs of patients recovering from injuries, surgeries, or chronic conditions in their own homes.
• Develop and implement individualized treatment plans based on best practices and patient-specific goals, incorporating various therapeutic techniques including manual therapy, exercises, and modalities.
• Monitor and document patient responses to treatments, adjusting care plans as necessary to ensure continuous improvement and goal attainment.
• Educate patients, families, and caregivers on therapeutic exercises, techniques for pain management, and the effective use of assistive devices to promote independence.
• Maintain up-to-date knowledge of advancements in the field by participating in professional development opportunities, including workshops and networking events.
• Collaborate closely with healthcare team members to provide holistic care that meets the physical, emotional, and social needs of each patient.

Patient Population / Client Focus

The ideal candidate will work with a diverse client population, treating a wide range of conditions from orthopedic injuries to neurological disorders. Our therapists engage with both pediatric and adult patients, tailoring interventions to support individual recovery goals, functional mobility, and overall well-being.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Current State License in Physical Therapy; Doctorate of Physical Therapy (DPT) preferred.
• Strong communication skills, both written and verbal, to effectively educate patients and collaborate with the healthcare team.
• Computer proficiency for documentation and patient management systems.
• Current CPR Certificate.

Preferred Experience
