Work Environment

This role is primarily based in a home care setting, allowing you the opportunity to engage with patients in their own environments—a key element in delivering effective rehabilitation. The work hours are generally Monday to Friday from 9 AM to 5 PM, providing a balanced schedule that promotes work-life harmony. Key Responsibilities

• Conduct comprehensive in-home physical therapy evaluations for patients recovering from conditions such as post-surgical procedures, orthopedic injuries, or neurological challenges.
• Develop tailored treatment plans based on thorough assessments, taking into account patients' needs, goals, and health conditions.
• Implement therapeutic exercises, gait training, and balance interventions that aim to improve functional mobility for patients.
• Educate patients and their families about home safety, fall prevention strategies, and overall wellness practices to ensure a supportive recovery process.
• Monitor patient progress meticulously, adjusting treatment plans as necessary to meet evolving functional goals.
• Maintain accurate and detailed documentation of all visits in compliance with Medicare and agency guidelines, ensuring streamlined communication and reporting.
• Collaborate with an interdisciplinary team, including nursing, occupational therapy, and speech-language pathology, to facilitate coordinated and effective patient care.

Patient Population / Client Focus

Your patients in this role will include diverse individuals, ranging from post-operative recovery cases to those needing rehabilitation due to chronic conditions. You'll have the chance to build meaningful relationships with your patients, understanding their unique circumstances and challenges, which will enable you to deliver personalized care that leads to successful outcomes.

Qualifications

• A valid and active Physical Therapist License in New Jersey.
• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid driver's license, a reliable vehicle, and appropriate auto insurance for travel to multiple patient homes.
• Current CPR/BLS certification.
• Exceptional clinical judgment, time management skills, and the ability to work independently in a home health setting.

Preferred Experience

• A minimum of two years of experience in physical therapy, ideally within home health, outpatient, or hospital-based rehabilitation.
• Familiarity with electronic medical record (EMR) systems such as HomeCare HomeBase, Kinnser, or MatrixCare.
• Understanding of Medicare documentation and OASIS regulations is preferred, although training will be provided.

Compensation and Benefits

We offer a competitive salary ranging from $90,000 to $110,000 annually, along with a comprehensive benefits package that includes health, dental, and vision insurance, 401(k) plans with employer match, paid time off, and holidays. In addition, our team members are eligible for mileage reimbursement or a company car depending on their role.
