Laurel Wood/Skyland Apartments are part of Volunteers of America National Services, which serves as the Housing and Healthcare affiliate of the Volunteers of America parent organization. Service Coordinator Job Highlights: Shift: Mon-Fri 8:00am-5:00pm. This position is 40 hours per week. 24 hours (3 days a week) at Laurel Wood and 16 hours (2 days a week) at Skyland. Pay: $26.00 per hour based on years of experience About the job: The Service Coordinator provides general case management (including intake) and referral services to all residents needing such assistance. Formal case management (i.e., evaluation of health, psychological and social needs, development of an individually tailored case plan for services, and periodic reassessment of resident’s situation and needs) for a resident is needed when such service is unavailable through the general community. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. 1. Bachelor’s Degree-concentration in Social Work, Gerontology, Psychology or Public Health preferred. 2. Training in the aging process, elder services, disability services, drug and alcohol abuse and mental health issues. 3. Proven experience in service management, including organizing, problem solving, and advocating. 4. Knowledge of community services in the region, particularly with services that are provided for the population living within the facility. 5. Knowledge of computer operations, to include basic data entry. 6. Ability to communicate effectively and maintain good relations with residents and employees. 7. Aware of eligibility for and procedures of federal and state entitlement programs. 8. Good communication, writing, problem solving and organization skills in addition to strong advocacy capabilities. ESSENTIAL FUNCTIONS Assists and advises residents and families of the services which may be necessary to maintain a self-reliant lifestyle. Promotes wellness activities for all residents. Educates residents, families, and staff on available community resources. Assists residents in building informal support networks among themselves and with family members. Acts as a liaison between community agencies, service providers, and residents. Works as a team member with Community Administrator in serving to project residents (follows up on resident situation the manager(s) believe may need supportive services). Encourages residents to be proactive in meeting their social, psychological, and physical needs. Facilitates meeting of needs when necessary, but avoids the creation of unhealthy dependence. Uses the least drastic intervention necessary to alleviate a problem situation. May assist residents or coordinate training for residents in understanding leases and tenancy obligations. Monitors the delivery of services to residents to ensure they are appropriate, timely and satisfactory. May also manage such services when appropriate. Performs service management function for all residents needing assistance. Provides limited case management, (i.e. evaluation of social, psychological and physical needs and the development of a service plan) for a resident when such service is not being provided by the general service community. Educates residents on service availability, application procedures, resident rights, etc., both individually and as a group. Reports all suspected abuse situations to the appropriate agency and Community Administrator. Travel Required Yes. Optional travel for annual training.
