Key Responsibilities

• Conduct thorough evaluations of clients' physical capabilities and treatment needs, ensuring an individualized approach to therapy.
• Create and implement comprehensive, personalized treatment plans designed to enhance strength, mobility, and daily functional independence.
• Utilize therapeutic exercises, manual therapy techniques, and electrotherapy modalities as appropriate to address specific physical impairments.
• Regularly document client progress using electronic health record systems and adjust treatment plans based on outcomes and feedback.
• Foster collaborative relationships within an interdisciplinary team, working closely with physicians, nurses, and other healthcare providers to develop cohesive care plans.
• Provide education to clients and their families regarding home exercise programs and strategies for maintaining physical health beyond therapy sessions.
• Adhere to all state and federal regulations, as well as established professional standards to ensure high-quality client care.

Patient Population / Client Focus

The primary focus of our Home Care services is working with the elderly and individuals recovering from surgery, injury, or chronic health conditions. Given the character of this role, you will often support clients who may exhibit a range of physical impairments, mobility limitations, or require post-operative rehabilitation. Understanding the unique challenges of this demographic is essential for tailoring effective treatment plans that address both physical and emotional needs.

Qualifications

• Current licensure as a Physical Therapist in New York State.
• A Bachelor's Degree in Physical Therapy from an accredited institution is required; however, a Master's or Doctorate is preferred.
• Strong communication skills, both verbal and written, to ensure clear interaction with clients and team members.
• Excellent organizational and interpersonal skills that facilitate the efficient management of caseloads and therapy sessions.
• Ability to adapt treatment methodologies based on individual client progress and situational needs.

Preferred Experience

• A minimum of two years of experience as a licensed Physical Therapist is required, with a preference for those who have worked in home health or skilled nursing settings.
• Experience working with geriatric populations is highly desirable, as well as familiarity with various therapeutic modalities and evidence-based practices.
• A commitment to ongoing professional development and education in the field of physical therapy.

Compensation and Benefits

The successful candidate will receive a competitive salary ranging from USD 90,000 to 110,000 annually, depending on experience.
