Work Environment

This position is situated in a home care setting, where you will work directly with clients in their residences across Albany. Your role will demand flexibility and adaptability as you create strategies to enhance mobility and therapeutic outcomes in various home environments. You will join a supportive team including two other physical therapists and a physical therapy assistant, where you can share knowledge and learn from one another.

Key Responsibilities

• Assess and evaluate clients' conditions to develop tailored treatment plans aimed at achieving their unique rehabilitation goals.
• Implement and conduct therapy sessions, ensuring that each client receives personalized and goal-oriented treatment designed to enhance mobility and reduce pain.
• Manage a case load of one patient per hour efficiently to maximize productivity while maintaining quality care.
• Utilize evidence-based practices and methodologies to deliver effective therapy interventions, including manual therapy and therapeutic exercises.
• Document patient progress through thorough record-keeping techniques and maintain compliance with all regulatory requirements.
• Collaborate with multidisciplinary teams, including physicians and other healthcare professionals, to provide holistic care and effectively address the needs of each patient.
• Motivate and encourage patients throughout their rehabilitation process, aiming to inspire lifelong adherence to wellness practices.

Patient Population / Client Focus

Your primary focus will be on patients with diverse needs, including those recovering from orthopedic surgery, injuries, and chronic pain conditions. We cater primarily to clients with private medical insurance, ensuring that your treatments are accessible and tailored specifically to their insurance coverage and rehabilitation needs. The aim is to minimize reliance on medications and surgical interventions, promoting functional independence.

Qualifications

• Licensed Physical Therapist in New York State with a Doctorate in Physical Therapy (DPT) qualification.
• Commitment to maintaining a clean record free of board actions or disciplinary measures.
• Strong background in manual therapy techniques and a proven ability to design and implement effective exercise regimens.
• Proficiency in EMR software platforms, preferably WebPT, for efficient patient management and documentation.

Preferred Experience

• Previous experience in orthopedic outpatient settings is highly desirable.
• Demonstrated ability to develop personalized wellness and fitness programs tailored to individual needs.
• Experience in a home care setting is a plus, as it will facilitate a smoother transition into the role.

Compensation and Benefits

The salary for this position ranges between USD 90,000 and 100,000 annually, commensurate with experience and qualifications.
