Key Responsibilities\n

• Conduct thorough bio-psychosocial evaluations to assess patients' mental health needs and eliminate barriers to treatment.

• Develop individualized treatment plans based on clinical assessments, employing evidence-based treatment modalities to address patient goals.

• Provide compassionate outpatient mental health therapy, utilizing both individual and group treatment approaches to support patient progress.

• Maintain accurate and comprehensive documentation of social work activities in patients' medical records, adhering to departmental standards.

• Consult with medical and support staff to coordinate comprehensive care programs designed to enhance patient treatment compliance and overall satisfaction.

• Educate patients and families about mental health conditions, recommended treatment strategies, and available community resources to support recovery and optimum functioning.

• Advocate for patients' needs and coordinate with community resources to ensure essential support systems are in place.

• Stay current on the dynamics of community resources and actively integrate them into treatment planning for patient benefit.

Patient Population / Client Focus\n

The LCSW role focuses on a diverse patient population, including children, adolescents, and adults facing a variety of psychosocial challenges exacerbated by mental health or physical illness. Qualifications\n

• A Master's Degree in Social Work from an accredited institution and a current state license as a Licensed Clinical Social Worker (LCSW).

• A minimum of six months clinical experience, preferably within a health or mental health setting, focusing on populations similar to those served by our clinic.
