This role will primarily be conducted remotely, allowing for flexibility and facilitating a work-life balance. Key Responsibilities\n

As a Licensed Clinical Social Worker (LCSW) at Grand Counseling Center, your responsibilities will include:

• Conducting comprehensive clinical assessments of hospice patients to understand their mental health needs and determine appropriate interventions.

• Developing individualized treatment plans tailored to the needs of each client, utilizing a combination of psychotherapy modalities such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT).

• Providing therapeutic interventions, navigating complex emotional landscapes related to end-of-life issues and grief, to help clients and families cope effectively.

• Maintaining accurate and secure clinical records while adhering to HIPAA regulations and ethical standards, ensuring the confidentiality of patient information.

• Working collaboratively with a multidisciplinary team of healthcare professionals to create a holistic approach to patient care, ensuring all aspects of a patient's well-being are addressed.

• Engaging in regular case consultations and peer review meetings to uphold clinical quality and share insights that can enhance service delivery.

• Offering education and resources to families, empowering them to understand the mental health aspects of terminal illness, and guiding them in utilizing available community resources.

Patient Population / Client Focus\n

Your primary focus will be on hospice patients who are facing significant psychological and emotional challenges. This vulnerable client population requires a high level of care, empathy, and targeted therapeutic interventions to help them navigate their end-of-life experiences with dignity and support. You will also engage with families, aiding them in understanding and coping with the dynamics of terminal illness and grief.

Qualifications\n

• A Master's degree in Social Work from an accredited educational institution.

• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in California.

• A minimum of 3 years of clinical experience in mental health settings, specifically providing psychotherapeutic services.

• Proficiency in solution-based interventions and therapeutic techniques relevant to hospice and end-of-life care.
