We are seeking a Per Diem Nurse Practitioner The Nurse Practitioner will work in accordance with the policies and procedures of Journey Hospice, and all applicable state and federal regulations. The role is under the general supervision of and in conjunction with the Hospice Medical Director. Responsibilities include performing patient medical examinations and palliative care visits required to certify patient eligibility for the Medicare Hospice Benefit after six (6) months of hospice care. The Nurse Practitioner documents visits in the patient’s clinical record and completes required forms for face‑to‑face encounters. Tasks are performed on an as‑needed basis, and the Nurse Practitioner communicates effectively with the Medical Director, Administrator, and the Hospice Interdisciplinary Team. Basic Qualifications Education/Training/Certifications: Graduation from a master’s‑level program designed to educate and prepare nurse practitioners/clinical nurse specialists. Completion of a graduate‑level 3‑credit course in Pharmacology from a school duly accredited by a national accrediting agency approved by the Board, or a pharmacology course of at least 45 hours integrated into the Master’s in Nursing program. National certification requires the candidate to successfully pass an examination. Current license and national certification via the PA Board of Nursing as an Advance Practice Nurse in PA. Valid driver’s license and reliable transportation that is insured in accordance with PA requirements. Skills Provide services in a timely and professional manner, adhering to hospice bylaws, operational policies, billing procedures, Medicare Conditions of Participation, PA Licensure Guidelines, and all applicable laws and regulations. Demonstrate a positive, professional manner, manage time efficiently, work collaboratively, and act as a role model. Develop and maintain effective relationships with the interdisciplinary group, community professionals, and physicians. Maintain a compassionate attitude toward caring for hospice patients. Communicate effectively and courteously with patients, caregivers, colleagues, supervisors, and all others. Show optimism, enthusiasm, and willingness to work constructively with other health care team members. Interact positively with all staff, supporting the mission and vision of the organization. Experience Minimum of two (2) years as a Nurse Practitioner. Understanding of the hospice program of care. #J-18808-Ljbffr
